A 50 year female is being operated for laparoscopic assisted vaginal hysterectomy (LAVHI) Suddenly the anesthetist noticed that oxygen saturation (SpO2) has started to fall, patient start becoming slightly cyanotic and there is increased resistance on mechanical ventilation and there is emphysema in neck. The diagnosis is:

Pneumothorax, pneumomediastinum and pneumo 'pericardium by co2 may occur during laparoscopy. The presentation is persistent hypoxemia (mayor may not associated with cyanosis), increased airway resistance decreased air entry on auscultation. X-ray chest<confirms the diagnosis.
