Schizophrenia and Other Psychosis
a. Late onset schizophrenia is after 45 year of age. Also ,called as paraphrenia.
b. Predominantly the delusion and Hallucinations, MC type of schizophrenia is paranoid type in late onset variety.
c. It is usually good prognosis schizophrenia and auditory hallucination (3rd person type is most characteristic)paraphrenia is characterized by the preoccupation with one or more semi systematized delusions.
d. These delusions are not encapsulated from the rest of the personality as in delusional disorder.
e. The affect is notably well-preserved and appropriate, as is the ability to maintain rapport with others. There is no intellectual deterioration, no flat nor grossly inappropriate affect.
f. Disturbance of behavior is understandable in relation to the content of the delusions.
g. The illness is associated with distress and agitation. Irrational behaviour may appear as delusions become more vivid and judgment lessens.
h. Patients may accuse others of persecution and complain to the authorities.
i. The predisposing factors in the onset of the disease are associated with severe stressors such as social isolation, migrant status and deafness.
j. This was first described by Emil Kraepelin.
k. The usual age of onset of schizophrenia is ≈ 20 years or late teens or late adolesnce.
l. Early onset schizophrenia is usually seen in hebephrenic or disorganised schizophrenia where onset occur is ≈ 13 to 14 years.
