Question
15 out of 17

A patient presents with acute renal failure with normal ultrasound abdomen report. Next most useful investigation is : (AIIMS Nov 09)

A Intravenous pyelography

B DTPA scan

C Retrograde pyelography

D X-ray abdomen and CT scan.

Ans. B DTPA scan

Imaging studies-

A. Radionuclide Studies-- Radionuclide studies can measure renal function.

a. Technetium diethylenetriaminepentaacetic acid (99mTc – DTPA) (dynamic renal scan) is freely filtered by the glomerulus and not reabsorbed and is used to estimate GFRQ.

b. Technetium dimercaptosuccinate (99mTc – DMSA) (State renal scan) is bound to the tubules and provides an assessment of functional renal mass.

c. Radioiodinated (131I) orthoiodohippurate is secreted into the renal tubules and assesses renal plasma flow (RPF).

Extra Edge:: Ultrasonography

A. Kidney size can be determined; a kidney less than 9 cm in length indicates significant irreversible renal disease.Q

B. A difference in size of more than 1.5 cm between the two kidneys is observed in unilateral renal disease.

C. Intravenous Urography (IVP)

Rapid sequence IVP is done for Renal artery stenosis
