Enterotoxigenic Escherichia coli (ETEC)
a. Endemic in developing countries among all age groups.
b. Major cause of traveler’s diarrhea
c. A large inoculum (106–1010 CFU) is needed to produce disease. Hence no human to human or via fomite transmission
d. Toxin production alone may not lead to illness, but it’s the main pathogenesis.
e. Disease is mediated primarily by a heat-labile toxin (LT-1) and/or a heat-stable toxin (STa) that causes net fluid secretion via activation of adenylate cyclase (LT-1) and/or guanylate cyclase (STa) in the jejunum and ileum.
f. The result is watery diarrhea accompanied by cramps.
