All are traumatic asphyxia except (AIPG 2010)

A Railway accident

B Road traffic accident

C Accidental strangulation

D Stampede

Ans. C
Accidental strangulation.
Traumatic asphyxia results from respiratory arrest due to mechanical fixation of the chest, so that the normal movements of the chest wall are prevented. Fatal cases are only due to accident. Usually there is a gross compression of the chest by a powerful force. Causes:-

a. Stampede,

b. Fall of earth or stones over the chest,

c. Compression against ground by weight of a motor vehicle,

d. Crushing by weight of a person as in a case of assault,

e. Industrial disasters, earthquakes, landslides,

f. Indirect compression of the chest as in a “jack-knife” position.

In all of the above options, chest can be compressed or mechanically “fixed” in- railway accident, road traffic accident or an stampede. But in a case of accident strangulation it is the neck which is compressed and not the chest.

Railway accidental injuries are primary injuries seen on the front or back of the body, secondary injuries are produced due to the body being thrown or run over which are soiled with oil and dust.

Road traffic injuries could be-

a. Injuries to pedestrians

b. Injuries to Cyclists & motor cyclists

c. Injuries to occupants of a motor vehicle.

Accidental strangulation- causes are

a. childen during play met get entangled in ropes, window cords, etc

b. infants caught in their cots when the neck is compressed in bars, restrainers braces, etc.

c. person may be strangled by a tight scarf or necktie specially if he is under the influence of alcohol, epilepctics or imbeciles

d. in case of movement of a fetus due to a cord around the neck

e. in a rickshaw due to the ‘chunni’ or ‘dupatta’ around the neck of the victim getting strangled in the wheel of the rickshaw.
