All of the following are true about anticonvulsants except (DNB Dec 2012)

A Phenytoin increases metabolism of other drugs by inducing hepatic enzymes

B Phenytoin causes gum hyperplasia

C Valproate is used in absence seizures

D Valproate stimulates GABA transaminases

Ans. D Valproate stimulates GABA transaminases

Valproic acid or sodium valproate is a broad-spectrum anticonvulsant. Remarkably, at anticonvulsant doses, valproate produces little sedation or other central effects. It is effective in partial and generalized tonic-clonic as well as absence seizures. Valproate appears to act by multiple mechanisms, which are enlisted below:

  1. A phenytoin like frequency dependent prolongation of sodium-ion channel inactivation;
  2. Attenuation of calcium-ion mediated 'T' current like ethosuximide;
  3. Augmentation of release of inhibitory neurotransmitter GABA by inhibiting its degradation by GABA transaminase as well as probably by increasing its synthesis.
