Silicosis is caused by inhalation of silica particles.

Clinical features: Progressive dyspnoea, incidence of TBQ,

CXR shows diffuse miliary or nodular pattern in upper and mid-zones and egg-shell calcificationQ of hilar nodes.

Spirometry: restrictiveQ defect

Important Points:

a. Silicosis has predilection for upper lobes

Coal worker’s pneumoconiosis (CWP) results from inhalation of coal dust particles (1-3 micro m in diameter) over 15-20yr. These are ingested by macrophages which die, releasing their enzymes and cause fibrosis Q.

1). CXR: many round opacities (l-10mm), especially upper zone Q.

2). Progressive massive fibrosis (PMF) is due to progression of CWP, which can cause progressive dyspnoea. fibrosis, and eventually, cor pulmonale Q.
