Optic neuritis - clinical signs

1. Visual acuity - usually reduced markedly (6/60 or less)

2. Colour vision - is often severely impaired

3. Ophthalmoscopic findings - Hyperaemia of the optic disc, and blurring of the margins, physiological cup is obliterated splinter haemorrhages and hard exudates may be seen

a. Inflammatory signs may also be present in the surrounding retina when papillitis is associated with macular star formation and the condition is labelled as "neuroretinitis"

4. Pupil shows ill sustained constriction to light, Marcus Gunn pupil which Q indicates relative afferent pupillary defect is a diagnostic sign

5. Visual field defect - The most common filed defect in optic neuritis is a relative central Q or centrocecalQ scotoma

6. Visual evoked response (VER) shows reduced amplitude and increased latinay

ERG is normal in disease involving ganglion cells and the higher visual pathway, such as optic atrophy

Clinical application of ERG

1. Diagnosis and prognosis of retinal disorders such as retinitis pigmentosa, Leber's congenital amaurosis, retinal ischaemia and other chorioretinal degeneration

2. To assess retinal functions when fundus examination is not possible ego (in the presence of dense cataract and corneal opacity)

3. To assess the retinal function of babies where possibilities of impaired vision is considered in ERG
