Poly cystic ovarian disease - characterized by oligo-amenorrhoea, obesity, hirsutism and an often infertility

a. Ultrasound reveals the ovarian morphology clearly and diagnosis can be accurately established.

b. High LH/FSH ratio (more than 1) results in anovulation

c. High oestrogen level but absence of progesterone

d. High levels of androgens [T, AD and DHEA)

Treatment

a. If the patient wishes to become pregnant - clomiphene, dexamethasone

b. If the patient does not desire pregnancy –Medroxyprogesterone

c. Hirsutism managed with epilation and electrolysis, spironolactone

d. If hirsutism is severe - Hysterectomy and bilateral oophorectomy followed by estrogen replacement therapy.
