a. The current National Cholesterol Education Program Adult TreatmentPanel III guidelines recommend specific target levels of LDLcholesterol LDL-C and HDL cholesterol HDL-C for determiningcardiovascular disease CVD risk and evaluating the effectivenessof lipid-lowering therapies.
b. While there is a growing consensusthat levels of apolipoproteinapo B and the ratio of apo B/apoA-I are more accurate predictors of CVD risk, the question hasbeen raised as to whether it is realistic to expect patientsand health professionals to switch from cholesterol-based guidelinesto apolipoprotein-based guidelines.
c. Because it will take timebefore apolipoprotein terminology is recognized by the generalpublic and recommended by the NCEP Adult Treatment panel toevaluate risk, it may be more efficacious to continue adheringto the already familiar and proven measurements of the LDL-C/HDL-Cratio.
d. The following review provides evidence that the LDL-C/HDL-Cratio continues to be a valuable and standard tool to evaluateCVD risk in all populations.
