Indications for surgery:
1). Recurrent pneumothorax
2). Isolated bullous disease
3). Lung volume reduction surgery (selected patients).
Guidelines for Referral and Transplantation for COPD (Ref. Hari-18th ed., Pg- 2190, table 266.1)
1). Referral BODE index >5
BODE index 7–10 or any of the following criteria:
a. Hospitalization for exacerbation, with PaCO2 >50 mmHg
b. Pulmonary hypertension or cor pulmonale despite oxygen therapy
c. FEV1<20% with either DLCO <20% or diffuse emphysema

1). Bupropion is a non-nicotine agent that is indicated as an aid to smoking cessation.
2). Varenicline is a nicotinic receptors agonist and is used for treating nicotine addiction.
