Clinical features of hyperthyroidism

1). Goiter: There is diffuse thyroid enlargement. ± bruit',

2). Gastrointestinal

Weight loss despite normal or increased appetite'

Diarrhea, Alimentary glycosuria can occur.

a. "Cardiorespiratory”

i. Palpitations', sinus tachycardia, atrial fibrillation', dancing carotid

ii. Increased pulse pressure, soft systolic murmur.

(Note: diastolic murmur is not a feature)

iii. Angina, cardiomyopathy and cardiac failure

iv. Means Lerman sound (it is scratchy sound heard over the precordium)

b. Neuromuscular

i. Nervousness, irritability, emotional liability', psychosis

ii. Tremor

iii. Hyperreflexia

iv. Muscle weakness, Hypokalemic Periodic paralysis, bulbar myopathy

c. Dermatological

i. Palmar erythema, spider naevi, Onycholysis, Alopecia , Pigmentation, vitiligo'

ii. Finger clubbing' (Thyroid acropachy), Pretibial myxoedema'

d. Reproductive

i. Amenorrhea/oligomenorrhea

ii. Infertility, spontaneous abortion iii. Loss of libido, impotence
