Tachycardia increases the oxygen demand of heart so anything that causes tachycardia (like atropine) is not at acceptable. Management of ischemia includes:
a. IV nitroglycerine (mainstay of treatment).
b. Heparin (inspite of increasing the risk of bleeding).
c. P blockers.
d. Calcium channel blockers.
e. Intraaortic balloon pump.
f. Cardiopulmonary bypass (patients going for CABG surgery).
