i. Beri-Beri disease is due to Thiamine deficiency

ii. Thiamine diphosphate (TPP), the active form of thiamine, acts as co-enzyme for several reactionsthat cleavage-carbon-carbon bonds:

a. The oxidative Decarboxylation of a (Pyruvate and a and Keto analogue of Leucine, Isoleu|ine and valine, and

b. Transketolase reaction of HMP shunt.

The most reliable (to detect deficiency of thiamine ie. Beri-Beri) is measurement of whole blood or RBC Transketolase activity. Any enhancement in enzyme activity resulting from added TPP, is referred as TPP effect. If the activity of the enzyme is increased> 15%, then a deficiency state is probably present.

c. Pyridoxine (B6):

i. Pyridoxal phosphate acts as co-factors for — Transaminase synthetases and Hydroxylases.

ii. Estimation of vit (B6) deficiency: -

A. Loading test:

i. Measurement of the excretion of tryptophan metabolites (xanthurenic acid) after tryptophan load.

ii. Cystathionine can be assayed after a methioine load

B. In vitro-measurement of RBC glutamic pyruvic transaminase, may a better indication of pyridoxine

states than either loading test
