Interpret the following data: paCO2 = 40, paO2= 90, Na= 135, K=5, HCO3= 23, Cl= 110, Calculate the anion gap. In which of the following conditions. The above anion gap can occur (PGI May 2010)



A RTA
B Lactic acidosis

C Multiple myeloma
D Salicylate poisoning

E Methanol poisoning

Ans. C

Multiple myeloma

In this case anion gap is (135) – (110 + 23) = 135 – 133) = 2.

So anion gap is much less than the normal (6 to 12).

Anion gap less than normal is seen in multiple myeloma. Hence C is the answer.

In RTA anion gap is normal and in option B and D anion gap is increase.
