Ketoacidosis without glycosuria is seen in

A Aspirin poisoning
B Renal tubular acidosis

C Prolonged starvation
D Paracetamol poisoning

Ans. C Prolonged starvation

Metabolism in starvation : three stages (Glucagon/Insulin Ratio )

a. 1st stage

i. Initially energy from blood glucose and stored glycogen it cannot last long

ii. Then glucose synthesized by gluconeogenesis (Insulin deficiency and glucagon excess

iii. responsible for this) this initial stage lasts for not more than 2 to 3 day is.

iv. Blood sugar falls (40-60 mg%) [ <40mg% is called Hypoglycemia -

b. 2nd Stage

i. 80-90%- of energy requirement will be derived from fats and the remainder 10-20% from

proteins

ii. This stage last> 2 weeks.

iii. Since due to excess of glucagon and deficiency of Insulin — Activates the Hormone sensitive

iv. lipase ed Lipolysis β-oxidation of FFAKetone formation (Ketogenesis

c. 3rd stage Energy obtained from protein break down. This stage last for less than 1 weak; now, we are clear, from above, during prolong starvation. There is ketogenesis and blood glucose level is maintained near normal, therefore no glycosuria (not hyperglycemia).
