It is a statement communicated by a doctor to the notifying or concerned authority due to his duty to protect the interest of the community or of the state. 
Exceptions of professional secrecy are included here. They are

  5. Courts of law.
  6. Servants & employer.

  7. Negligence suits / Notifiable diseases.  
Privilege communication is not mandatory by the doctor except in situation where it is required by law or public health authorities to inform the police, magistrate or relevant authority.
