The most common cause of mismatch transfusion (ABO incompatibility) is clerical error. In conscious patient I it may present as fever with rigors and chills, nausea, vomiting, flank and chest pain, dyspnea and tachypnea but under anaesthesia the usual presentation is persistent hypotension and increased oozing from surgical site (increased bleeding is more specific sign than hypotension).
