Nitroblue tetrazolium test

a. Normally phagocytic cells (monocytes, macrophages and neutrophils) produce superoxide free radicals by NADPH oxidase to kill phagocytosed organism.

b. These superoxide free radicals directly reduce nitroblue tetrazolium into an insoluble form à positive test in normal individuals.

c. In chronic granulomatous disease, these superoxide free radicals & H2O2 are not produced, so that nitroblue tetrazolium is not reduced à negative test in chronic granulomatous disease.
