Increased requirement of sedation and muscle relaxant
SIMV is the synchronized mode in which ventilator synchronizes with the patient breath and will deliver between the patient breaths. The advantages over controlled mode ventilation are:
a. Less hypotension and less decrease is cardiac output.
b. Less requirement of sedatives and muscle relaxants (in CMV patient has to be completely paralysed because ventilator does not synchronize with patient breath).
c. Less dead space so less V/Q mismatch.
d. Since patient is participating in respiration so weaning is rapid.
The disadvantage is that increased work of breathing can cause muscle fatigue.
