Rx for sputum positive pulmonary TB in First trimester (AIIMS Nov 12)

a. All sputum smear positive new cases of pulmonary TB receive Cat 1 treatment and a pregnant woman is no exception.
b. In fact a pregnant woman with sputum positive TB should receive treatment as soon as possible so that before she delivers she is rendered sterile and no longer shedding mycobacterium in her sputum.
c. There is no rationale for withholding the treatment till the second trimester as none of the Cat 1 drugs have known teratogenic potential.
d. On the other hand a pregnant woman should never receive Cat 2 treatment(even if indicated) as it contains streptomycin is teratogenic which is contraindicated during pregnancy.
