When the involvement in Paget’s disease is wide spread the onset is so insidious that the actual beginning of the disease goes unnoticed. The initial complaint usually is pain at the site of involvement. When the pain is over the tibia the bone is found to be enlarged and bowed in anterolateral direction the so called saber shin. The skin over the subcutaneous aspect of the bone is warm to touch but the bone is not tender.
