Sulphuric Acid

  1. Brown to black streak from angle of mouth
  2. Brown to black vomit
  3. Brown to black stomach wall

  6. Blotting paper appearance of stomach mucosa
  7. Black spongy mass is what stomach looks on PM
  8. Ulceration of esophagus (fibrosis & stricture )
  9. Perforation of stomach:
    1. T/t - gastric lavage contraindicated
    2. give water or milk with CALICIUM or MAGNESIUM OXIDE ( then nothing orally )
    3. Bicarbonate is contraindicated, it liberates co2 which can come gastric dilatation rupture of stomach
    4. SKIN BURN : water + paste of magnesium oxide or sod.  Bicarbonate
    5. EYE BURN :irrigate with water or sod. Bicarbonate

Vitriolage

  1. Throwing sulphuric acid on face
  2. t/t - wash with water or sodium bicarbonate apply a paste of magnesium oxide
  3. Gastric lavage is contraindicated as in other corrosive acid poisoning ( except Carbolic acid). Lavage is also contraindicated in Kerosene & Volatile poison.

Nitric Acid

  1. Xanthoproteic reaction: YELLOW discoloration of tissues due to production of picric acid
  2. yellow teeth
  3. yellow stains on clothing
  4. yellow staining of tissues, the PM app.
  5. but the stomach is BROWN
  6. s/s - same as sulphuric acid
  7. t/t - same as sulphuric acid
  8. fatal dose – 10 – 15ml (same as sulphuric acid)
  9. fatal period – 12 – 24 hrs (same as sulphuric acid)

Carbolic Acid

  1. Phenol (synonym of carbolic acid )
  2. Pyrocatechol & hydroquinone are partially oxidized product of phenol in the body.
  3. Putrefaction is delayed
  4. PCT necrosis , heamoglobinuria & albuminuria ( haemorrhagic nephritis is PM app.)
  5. Peripheral neuropathy; burning n numbness due to damage to nerve endings.
  6. Pulmonary and laryngeal oedema , respiratory center depressed , breathing is strenuous (complication is bronchopneumonia ), death due to asphyxia.
  7. Precipitation of protein & coagulation of cell contents
  8. White opaque eschar on the skin
  9. greenish White app. Of necrosed tissue
  10. White leathery app. Of stomach mucosa (PM app. )
  11. CARBOLURIA : greenish urine due to oxidation of Pyrocatechol & hydroquinone in the urine.
  12. OCHRONOSIS : pigmentation of cornea & cartilages due to Pyrocatechol & hydroquinone .
  13. T/T - water + animal charcol , castor oil , saccharated lime magnesium sulphated or paraffin left in the stomach.

Oxalic Acid

  1. Spinach , cabbage naturally contain it.
  2. Used in ink remover solution.
  3. Oxalates produces tubular necrosis & death due to uremia.
  4. s/s – Coffee ground vomitus
  5. hypocalcemia Calcium oxalate crystals , blood , albumin in urine
  6. death in 1 – 2 hrs , pulse is feeble & rapid ,if patient survives digestive upset occurs .
    1. t/t – Calcium lactate or gluconate for stomach wash
    2. Calcium gluconate 10% ** , 10 ml iv as antidote
    3. Chalk , lime water , Calcium chloride converts oxalic acid in to insoluble calcium oxalate.
    4. Parathyroid extract *
    5. PM APP. – black stomach *
    6. Brownish stomach content due to acid heamatin * formation
