Ans. A Has more side effects than penicillamine.

a. Trientine is structurally related to ethylenediamine. The drug has been used as an hardening agent in chemical reactions in the past.

b. Its hydrochloride salt is used as a copper chelator in treatment of Wilson’s disease. Though oral administration of this drug is poor, yet it is given orally.

c. D penicillamine is generally the first drug used in treatment. Side effects of this drug are considerable. Muscle weakness develops in 1/5 of cases. Many patients develop lupus like symptoms. Worsening of neurological signs too can occur.

d. Pemphigus like eruption is seen in few cases. Rare cases develop hepatic fibrosis. Zinc and trientine are not recommended during pregnancy, as they are teratogenic.

e. Liver transplant is the only cure for Wilson’s disease. This should be considered in severe or drug refractory patients. The drug can reduce absorption of iron and magnesium (BG Katzung’ 10th Edition, 2010).
