Diabetes insipidus

Two types. Neurogenic/ Nephrogenic


  1. Polyuria & polydipsia should be confirmed by 24 hours intake and output records.
  2. Daily volume of urine-4-10 litres
  3. Urine special gravity - 1001 -1005
  4. Urine osmolality-50-200 mosm 

Plasma osmolality is normal

  1. Water deprivation for a minimum of 3 hours
  2. Urine osmolality remains below plasma levels.
  3. Administration of desmopressin DDAVP (5 micro g intranasal) raises the urine osmolality.
  4. Failure to concentrate urine and absence of response to DDAVP indicate nephrogenic diabetes                                                                  


  1. Neurogenic DI: Desmopressin             
  2. Nephrogenic DI: Hydrochlorothiazide
