Pulsating exophthalmos is due to arteriovenous Q malformation in which communication takes place between the internal carotid artery and cavernous sinus. It also occurs in neurofibromatosis type I where these is a roof defect/ sphenoid wing dysplasia. In meningoencephalocele, there is orbital roof defect and there is transmitted pulsations.
