Shoulder pain post laparoscopy is due to:

|D||Compression of the lung|
a. Post-operative shoulder tip pain
b. This occurs in up to 40 % of patients after CO2 laparascopy and usually affects the right shoulder.
c. It is believed to be caused by the irritating effects of CO2 producing carbonic acid locally and affecting the peritoneal lining of the right diaphragm which in turn is referred to the shoulder.
d. Shoulder tip pain can be significantly reduced by using insufflation pressures of < 9 mm Hg.
