In chronic papilledema persistently elevated CSF pressure in the optic nerve produces stasis of axoplasmic flow and swelling of optic disc which eventually causes disruption of neurofilament and nerve fibre atrophy.
Initially there is development of axonal swelling in the prelaminar region which leads to blockage of axoplasmic transport. However there is no accumulation of extra cellular fluid.
